Transition Solutions provides safe, cost effective transportation from the hospital or rehabilitation facility to your home.
Our Caring Solutions caregiver will arrive at the time of your discharge, gather your belongings, and drive you home. Once at home, the caregiver will help with any housekeeping chores; shop for groceries; pick up your medications; or any other assistance that will make your arrival at home is an exceptional experience.
Our caregivers are fully screened, bonded, and insured. We verify the caregiver’s auto insurance coverage as well as ensure they have a clean driving record.
Services Include*
- Medication pickup
- Check the refrigerator for expired food
- Go to the store for or with the client
- Laundry, change linens
- Prepare a meal
- Help sort through and prioritize mail
*Time allotted is 3 hours, planned from scheduled pickup at the facility until caregiver leaves the client's home. If additional assistance is requested, additional charges will apply.
