Transition Solutions is a program offered by Caring Solutions, LLC to provide safe, cost effective transportation from the hospital or rehabilitation facility to your home. Our caregiver will arrive at the time of your discharge, gather your belongings, and drive you home.

Once at home, the caregiver can help with any housekeeping chores; shop for groceries; pick up your medications; or assist in any other way to make your arrival home an exceptional experience.

Our caregivers are fully screened, bonded, and insured. We verify the caregiver’s auto insurance coverage, as well as ensure that their driving record is in good standing.

Service Include*

• Transportation
• Medication pickup
• Check the refrigerator for expired food
• Go to the store for or with the client
• Laundry, change linens
• Prepare meals
• Help sort through and prioritize mail
• Other services as time allows

* Time allotted is 3 hours, planned from scheduled pickup at facility until caregiver leaves the client’s home. If additional assistance is requested, additional charges will apply.
